This plan offers significant savings to patients who may require more than one transfer to achieve a live birth. There are no medical pre-qualifications but the cost varies depending on the woman’s age at the start of first stimulation and the method of fertilization in the first cycle. The plan includes up to 2 egg retrievals and up to 4 embryo transfers (typically 2 fresh and 2 frozen embryo transfers, provided frozen embryos are available). All transfers must be completed within 12 months of the start of treatment.

What is included in the two-cycle plan:
- Monitoring with ultrasounds and blood tests at Alta Bates program
- Up to 2 egg retrieval fees including anesthesia and operating room
- Embryology laboratory including ICSI, hatching, extended culture and freezing as appropriate.
- Up to 4 embryo transfers with ultrasound guidance and post-transfer care.
What is not included in the two-cycle plan:
- Pre-treatment work-up (semen analysis, HSG, saline hysterogram, hormone tests, infectious disease tests, prenatal panel, genetic testing)
- Medications
- Donor sperm
- Pregnancy diagnosis and management
Deposit on the two-cycle plan:
The entire cost of the treatment must be paid to R.J. Chetkowski, M.D., Inc. in advance of the baseline ultrasound visit by check or cash.
Financing through the Capital One Family Fee Plan cannot be used for the two-cycle plan.

Medications
The cost of medications is not included in either one of the above payment plans and it varies widely depending upon the quantity required and price. Comparative shopping is often worthwhile. We provide you with prescriptions and a list of participating pharmacies where the medications can be obtained. Please make sure that you have the medications well in advance of your treatment cycle.
